When you identify a risk at your centre — whether spotted during a daily check, raised from an incident, or noticed during routine operations — you can log it directly in the hazard register. Logged hazards stay visible until they’re resolved, so nothing gets forgotten.

Who can log a hazard

Compliance users can add hazards to the standing register directly. Any staff member can flag a hazard through the daily check process — if a daily check item fails, it can be promoted into the register by a compliance user.

Adding a hazard

From the Hazard & Premises module, go to the Hazard Register tab and click Add Hazard. Fill in the following details: Title — a short, clear description of the hazard (e.g. “Loose paving near the sandpit”). Description — more detail about the nature and location of the hazard, and any relevant context. Premises location — select the part of your centre the hazard relates to. Locations are set up in the Premises section of the module. Initial risk rating — your assessment of the risk before any controls are applied: Red (high), Amber (moderate), or Green (low). Control measures — record what actions have been or will be taken to address the hazard, using the hierarchy of control:
  • Eliminate — remove the hazard entirely
  • Minimise — reduce the likelihood or severity
  • Isolate — separate the hazard from people
Residual risk rating — the risk level that remains after your controls are applied. Owner — assign the hazard to the staff member responsible for managing it. Review due date — set a date for the hazard to be reviewed again. Click Save to add the hazard to the register.

Promoting a daily check finding

If a daily check item was marked as a fail, you can promote it to a standing hazard from the check record. The hazard form pre-fills the title and premises location from the check item — review and complete the remaining fields, then save. The link between the check finding and the resulting hazard is recorded automatically.

After logging

The hazard appears in the register with its initial and residual risk ratings displayed as colour-coded badges. It stays active until a compliance user marks it as Resolved. You can update any field on a hazard at any time — for example, to record new control measures, change the owner, or adjust the residual rating after an action has been completed.

Linking to an incident or maintenance request

If the hazard results in an incident, you can raise one directly from the hazard detail view. See Hazards & Premises for more on how the Hazard → Incident link works. If the hazard requires a maintenance job, you can raise a maintenance request from the same view, which creates a link through to the Maintenance Log.
